Contraindications (limited evidence)
- Hypothyroidism (may interfere with thyroid hormone action): L-Carnitine acts as a functional thyroid hormone antagonist at the receptor level; it inhibits thyroid hormone entry into cell nuclei, reducing gene transactivation by T3 and T4; subjects with hypothyroidism should be aware that L-Carnitine supplementation may worsen hypothyroid symptoms or reduce the effectiveness of levothyroxine therapy.
